Are you a qualified social worker looking for an exciting new opportunity?Due to career progression, we have a vacancy working within our Integrated Services for adults with disabilities based at the Millennium Centre in Leasowe Wirral.

Our team works with people with complex needs to support them, and their carers, to get the outcomes they need. CWP is at the forefront of health and social care integration and this service lends itself to developing new, innovative ways of working together.

You will be a person-centered practitioner and will have experience of carrying out Care Act assessments, developing personalized support plans, completing mental capacity assessments, and taking a lead on safeguarding enquiries. A good understanding of asset and strength-based approaches to assessment and support planning is vital. In return we can offer you a clear career progression framework with a range of training opportunities. You must be a qualified social worker and registered with SWE.

Main duties of the job

Essential

  • Social Work qualification e.g. Degree in Social Work; DipSW or equivalent
  • Be registered with Social Work England and able to evidence this
  • Evidence of continuing professional development in line with PCF
  • Required to undertake the practice educator award
