Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is looking to recruit a highly motivated and flexible Learning Disability Nurse who is interested in joining an innovative, community based LD CAMHS Team covering Cheshire. The base will be within Cheshire West but may include travelling across Cheshire, expenses reimbursed as per policy.

The post holder will work as part of the multi-disciplinary community Learning Disability CAMHS Team, and will be expected to work autonomously, managing a defined case load of 0-18 year olds with Severe Learning Disabilities and challenging behaviour.

The post holder will hold a generic caseload who may also present with:o complex physical health needso mental health difficultieso forensic propensity

The post holder is expected to work in 3 core areas:Direct clinicalTraining and development of othersProject work to help develop services offered to young people with learning disabilities.

Applicants are welcome from both LD nurses who have experience in adult or Children's work.

Main duties of the job

You would be joining an established Community Team consisting of Learning Disability Nurses, a Clinical Support Worker, an Assistant Psychologist, Clinical Psychologist and Psychiatrist. The post does require the ability to travel in a timely manner across the Cheshire area as needed.

The team offer evidence based behavioural and sleep assessments and interventions to children and young people with learning disabilities who display behaviours that people may find challenging.

You will be required to carry out the assessment of behaviours and develop person centred behavioural intervention plans for a caseload of young people between the ages of 0 -18, utilise a Positive Behaviour Support model therefore prior knowledge of this model would be extremely beneficial. You willbe required to provide group training to parents and professionals as needed.

You will be the responsible clinician for multiple cases and be required to complete all necessary documentation.

The post would require the ability to be able to work early mornings and evenings when required by the service.
