Job summary

**We offer a relocation package of up to £8,000**

The relocation package is for anyone joining the Trust in a Clinical role who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

An exciting opportunity has arisen for a full time Team Manager within our newly established, Discharge Facilitation Team. The successful applicant will be expected to work across Urgent Care Mental Health First Response Services, which will include Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services and the Crisis Line.

Main duties of the job

The successful candidate for this role will be responsible for the Management of our Discharge Facilitation Team.

You will be expected to manage, lead and be accountable for the provision of high quality effective leadership and management of the team and ensure an optimum level of safe and effective patient care within budgetary resources.

You must be able to develop the team with a multidisciplinary approach and using developed managerial and leadership skills to promote excellent working practice, promoting the rights of service users in line with national and regional strategies.

The post holder will be expected to work as part of the Urgent Care Mental Health First Response service, which includes Crisis Triage & Response Services (Crisis Line/First Response), Liaison Psychiatry, Crisis Resolution & Home Treatment Teams, Street Triage and Liaison & Diversion services (Criminal Justice).
