Do you want to work in a friendly, professional and patient-focused Community Mental Health Team where your skills, knowledge and experience would be valued?

If you are a mental health practitioner with an interest in delivering high quality patient care in the community, then you could be the person we are looking for.

We are after a band 6 mental health practitioner to join our team as working with client's who have complex mental health conditions and support them to achieve their individual, recovery goals. You would be working as an autonomous practitioner managing an individual caseload within the Care Programme Approach (CPA). The successful applicant would be responsible for assessing care needs and the development, implementation, and evaluation of proposed interventions.

The position is part time 30.0 hrs per week and is based at Jocelyn Solly Resource Centre. The service operates 9am-5pm, Monday to Friday and considerations can be made to flexibly delivering the part-time hours through the working week. You would be joining a well-established, experienced and supportive multi-disciplinary team that is always looking to evolve and adapt to changing practices in mental health care. The position includes actively engaging in service planning and development as well as focusing on personal growth and development.

Main duties of the job

To manage a defined caseload with responsibility for the review of care needs for service users.

Establishing a good rapport with clients and their families.

Delivering professional standards of care to the very highest using evidence based practice.

Assisting medical staff, and members of the primary care team, voluntary agencies and other professionals, to meet the needs of the people of Central / East locality To undertake and comprehensively complete regular reviews for identified caseload Participate in team meetings and review client care and needs with a senior members of the Older adults community mental health team, Facilitating effective delivery of the Care Programme Approach and engaging clients, families and carers with services.

Ensuring that all communications with colleagues, clients and families are clearly understood, giving written confirmation of verbal communications where there is observed to be a need. Interpreting Trust and National Health Service policies for clients and family and professionals as required. Attend MDTs Specialist CMHT's/case conferences/care programme approach planning and review meetings where appropriate.
