Job summary

As part of our community mental health transformation programme, Cheshire and Wirral Partnership NHS Foundation Trust (CWP) has been working in partnership with our Primary Care Networks to expand the primary care mental health practitioner roles. This is an exciting opportunity to be part of the primary care team within the Central Wallasey Primary Care Network (PCN).

We are working together to provide joined up services to give patients access to support to live happier, healthier lives. We are looking to recruit a new Primary Care Network Mental Health Practitioner to join our PCN team. Our multi-disciplinary team already includes GPs, Nurse Practitioners, Nurses, Healthcare Support Workers, Phlebotomists, Pharmacists, Pharmacy Technicians, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers and Physiotherapists.

Working in partnership, the role will be directly employed and supported by CWP, but fully embedded within the PCN multi-disciplinary team to develop new ways of working and providing that essential bridge between the PCN and CWP.

This is an exciting opportunity to work with a primary care setting and we are looking for enthusiastic and motivated people to apply for these posts.

Main duties of the job

On a day-to-day basis, PCN Practitioners will work as a member of GP practice extended teams and be based in practices, and will provide local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for patients presenting with mental health problems.

This role will also support secondary mental health services by acting as the interface from primary care to manage referrals into the correct services at the correct times.

The role will operate within the principles of recovery and be responsible for the case management of agreed individuals with mental health conditions in conjunction with the Primary Care Team, liaising with GP's and other allied healthcare professionals to manage patients with a wide range of presenting mental health conditions.

You will need to have good organisational skills as you will be managing a caseload and the provision of services to people with mental health care needs as well as offering support and advice to other health care professionals. You will be an autonomous practitioner, making clinical judgements, identifying alternative courses of action, managing and providing a high-quality care.
