Job summary

CAMHS Tier 4 New Model of Care is focused on reducing avoidable admissions, enabling shorter lengths of stay and decrease out of area placements. The New Care Model was clinically-led, co-produced and developed with all stakeholders including young people and families.

Main duties of the job

There are three core elements to the New Care Model:

  • A place-based Gateway meeting to ensure that all agencies take responsibility for young people at risk of admission to Tier 4 receiving inpatient mental health treatment
  • A risk Stratification Tool to provide an evidence based way of identifying young people at risk of admission.
  • Ancora CARE - a hospital at home service for young people across Cheshire and Merseyside.

This post will join the Ancora CARE service, who's aim is to support the reduction of admissions to Tier 4 inpatient beds, appropriately accelerate discharge and support the delivery of care close to home. This is an exciting time to join Ancora CARE with the innovative approach of the New Care Model.

Essential

  • Relevant professional qualification e.g. Mental Health Nursing, Social Work, Occupational Therapy
  • Professional knowledge acquired through training and experience in a mental health provision
  • Evidence of on-going professional development
  • Experience of working with complex systemic presentations

  • Ability to demonstrate ongoing learning and development.
  • Evidence of undertaking training in current therapeutic approaches
