Job summary

The MHIST team (a community rehabilitation team) work with the most complex individuals who are presenting with high levels of risks, vulnerabilities and/or challenges. It is values driven, diagnostically inclusive and work with all individuals - who meet the threshold (high levels of complexity) and where the team can add value.

The service delivers a function (intensive rehabilitation in the community) rather than being a parallel team in its own right. It scaffolds what other mainstream services provide and interlinks with the wider CMH transformation. The service sits within the PCN with region-wide core standards, offering equitable service to all PCNs with the golden thread of 'reducing health inequalities' running through all its facets

Main duties of the job

The role will be to work alongside the team:-

  • To help people to achieve and maintain stability of their mental health and to gain (or regain) the skills and confidence for successful community living
  • To meet people's clinical needs by providing specialist rehabilitation interventions
  • To reduce the need for hospital admissions and increasing stability in the lives of service users and their carers/family
  • To improve people's social and everyday functioning and autonomy by enabling self-management skills
  • To adopt a recovery based approach that instils hope and empowers people
  • To enable people to access the community assets that can enhance their social networks and social inclusion
  • To facilitate people to progress along the rehabilitation care pathway, from higher to lower supported settings
