Job summary

Do you want to work in a friendly, professional and patient-focused Community Mental Health Team where your skills, knowledge and experience would be valued? If you are a mental health practitioner with an interest in delivering high quality patient care in the community, then you could be the person we are looking for.

We are after a band 6 mental health practitioner (Qualified RMN) to join our team to work with client's with complex mental health needs and support them to achieve their recovery goals. You would be working as an autonomous practitioner, with input from the multi-disciplinary team, managing an individual caseload and assessing new referrals to mental health services. This involves developing, implementing and evaluating interventions for our clients.

The position is full time and is based at Jocelyn Solly Resource Centre. The service operates 9am-5pm, Monday to Friday and considerations can be made to flexible working. You would be joining a well-established, experienced and supportive multi-disciplinary team that is always looking to evolve and adapt to changing practices in mental health care. The position includes actively engaging in service planning and development as well as focusing on personal growth.

Main duties of the job

o Undertaking assessments and coordinating the care of service users.

o Ensuring that communication with the service users / families / and GPs is of the highest standard.

o To develop and maintain positive working relationships with service users, their carers, General Practitioners, third sector partners, other significant professionals, and members of the multi-disciplinary team.

o Ensuring that clear channels of communication are established and maintained.

o To promote and identify areas requiring development within the CMHT in the Cheshire East area and to take a lead role in the development of services in conjunction with the Team Manager, Head of Clinical Services and other professionals.
