Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is looking to recruit a highly motivated and flexible coordinator/case manager who is interested in joining an innovative, community-based LD CAMHS Team covering East and Central Cheshire. The base will be within Cheshire East or Central Cheshire and will be negotiated with the successful applicant.

The post holder will work as the Coordinator/Case Manager of the multi-disciplinary community Learning Disability CAMHS Team,

The post holder will be responsible for the coordination of the team and will hold a small generic caseload who may present with:o complex physical health needso mental health difficultieso forensic propensity

The post holder is expected to work in 4 core areas:

Leadership/managementDirect clinical workTraining and development of othersProject work to help develop services offered to young people with learning disabilities.

Applicants are welcome from LD nurses/Allied professionals who have experience in adult or Children's work.

This is a fixed term 12 month post to cover current post holder.

Main duties of the job

You would be joining an established Community Team consisting of Learning Disability Nurses, Clinical Support Workers, an Assistant Psychologist, Clinical Psychologist and Psychiatrist. To clinically and managerial lead the teams . The post does require the ability to travel in a timely manner across the Cheshire area as needed.

The team offer evidence based behavioural and sleep assessments and interventions to children and young people with severe learning disabilities who display behaviours that people may find challenging.
