Job summary

An exciting new opportunity has arisen for an experienced Occupational Therapist in Primary Care.

These roles have been created as part of the NHS Long-Term Plan and mental health community transformation to put clinicians with mental health experience directly within primary care networks. The funding for these roles is an exciting partnership between CWP and the PCNs as they are funded jointly between the two. The person will be directly employed by CWP but jointly managed and supported by CWP and the PCN in what will be a new, collaborative way of working.

This innovative role will be a developmental post, enabling the successful applicant to help embed the Occupational therapist role into primary care and enhance the mental health support and well-being service we offer. The post will cover 4 GP Practices: Boughton Health Centre, Park Medical Centre, Heath Lane Medical Centre and Upton Village Surgery .

The post holder will be pivotal in driving change for the delivery of Occupational therapy services in primary care for people experiencing mental health difficulties, drawing on and increasing the strengths and capacities of local communities.

Main duties of the job

You will be enhancing the service delivered by the multidisciplinary team by supporting patients to help better manage their own self-care, keep them well, and reduce visits to the practice the patient may otherwise have needed to make.

You will be helping to develop an innovative service in the primary care setting, focusing on early intervention for individuals with complex needs, preventing escalation of mental health problems, and wider deterioration of the individuals we support in practice.

In your role, you will be promoting the value of physical and mental health needs equally, for our patient population, the multi-disciplinary team, and the wider care community.

You will contribute to the development of new and innovative concepts, models, methods and practices in order to support/deliver new and improved proactive general practice and primary care services.

You will have the autonomy to deliver a new service that you will help design to holistically assess the patients' needs in a person centred manner, with your training and experience in addressing both mental health and physical health needs simultaneously.

Following assessment, you will focus on resolving health and social issues at an early stage, helping to minimise crisis situations that result in appropriate presentation or admission to residential or hospitalised care

Essential

  • BSc in Occupational Therapy.
  • Registration with Health Professions Council.
  • Accreditation as Practice Placement Educator.
  • Significant postgraduate training working towards a master's degree in a speciality relevant to the post.

Desirable
