Job summary

The opportunity has arisen for a motivated, enthusiastic and child focused person to join the Children's Community Nursing Team as a part time Health Care Assistant.Candidates must have excellent communication skills and demonstrate a compassionate and empathetic approach to supporting children and families.The successful applicant will assist the qualified nursing staff within the Children's Community Nursing Team to provide care and support to infants, children and young people up to 18 years in their own homes or other community settings. You will work autonomously in the communityfollowing training, within the level of competence for the post to deliver agreed plans of care. There will be opportunities to progress professionally supported by training and learning opportunities.Please indicate within your application whether you have a clean driving licence. Main duties of the job

The post holder:-

Will perform agreed plans of patient care to identified groups of children/ young people without direct supervision within the level of competency for the post

  • Act as a resource for families and children with health care needs.

  • Work collaboratively with the acute ward, GPs, HV's and other health professionals to enhance seamless care and reduce admissions to in patient wards

  • Monitor and ensure standards of care are maintained and that policies / procedures and protocols are adhered to

? Always maintain respect and confidentiality

  • Maintain clear and accurate documentation using an electronic system, raising issues of concern with a qualified nurse where appropriate

  • To encourage and maintain good relationship with the CCNT, Specialist Children's Hospitals, Hospices, Medical Staff, Community Nurse Specialists and AHP's in order to provide the best care.
