Job summary
The Nurse Consultant for Stroke services will lead the development of patient-focused, evidence-based practice, setting, maintaining, and developing professional standards for multi-professional practice to patients with strokes across the Trust services. This will include ensuring robust risk assessment and risk management processes are embedded in practice.
Work across professional and organisational boundaries, developing specialist knowledge underpinned by theory and practice, undertake advanced level assessment, and initiate clinical interventions in order to address complex needs for patients with strokes who have on-going physical health needs.
Main duties of the job
Work as an autonomous practitioner; providing clinical leadership through the development and delivery of the highest level of clinical practice, and provide expert nursing through direct care, consultancy, and clinical supervision of others.
Lead the development and implementation of patient-focused clinical pathways, ensuring evidence-based care is embedded within practice, incorporating any national guidance or policies, and benchmarking Trust services against the standards set out within Government documents and national strategies.
Develop and maintain strong working relationships with a wide range of statutory, voluntary and community organisations, and lead and participate in the work of interdisciplinary and multiagency groups.
Promote excellent in professional practice, contributing and leading on quality improvement, patient safety and clinical assurance by leading, engaging and facilitating teams of staff to deliver high standards of evidence-based care. This will include balancing the challenges of providing high quality, safe services, with excellent outcomes in a way that delivers efficiencies and meets the needs and expectations of patients, carers, and the public.
