Job summary

The post holder will work as in integral member of the Head and Neck Team, providing exemplary service for patients with Head and Neck and Thyroid cancer.

To provide specialist nursing care and Nurse led clinics and provide personalized care and support planning for patients with Head and Neck / Thyroid Cancer.

To provide a specialist nursing service undertaking assessment and triage of patients with a known or suspected cancer diagnosis, ensuring timely access of any required investigations/service interventions.

The post holder will provide advice and support to wider members of the multi-disciplinary team.

The post holder will have close liaison with the CNS team, AHP's, Consultants and will be expected to share nursing responsibility within the service, and in association with the Nursing strategy for the Trust.

Main duties of the job

  • To provide specialist nursing and to support the provision of care for patients with Head and Neck / Thyroid Cancer at all points along the care pathway.
  • To liaise and work alongside the appropriate multi-professional teams to support patients with complex needs.
  • Act as a clinical expert/advice resource for patients, families, carers and health professionals about diagnosis, treatment, follow-up and survivorship.
  • To ensure that the key worker role and holistic needs assessment are identified and undertaken at appropriate points of the patient care pathway in liaison with other site specific multi-professional teams.
  • To practice nursing care within the Head and neck team, ensuring appropriate investigations, diagnosis are received in timely manner to achieve 62 day pathway.
  • To attend and when needed, present to the Head and Neck Cancer Site MDT meetings.
  • To maintain accurate, appropriate and concise patient records and other appropriate records, utilising PPM and EPR.
  • To provide relevant and clear information, education and advice for patients, carer's and family at all times.
  • To disseminate information on education and training
  • To maintain links with Macmillan service development
  • To act as the patient's advocate, supporting their involvement in decision making.
