Job summary

We have a very exciting opportunity for a pharmacist to contribute to the clinical pharmacy service at Calderdale and Huddersfield NHS Trust (60% of the time) in conjunction with developing themselves as a teacher practitioner, working with undergraduate MPharm students at the University of Huddersfield (40% of the time).

The post holder will be expected to demonstrate a high standard of patient focused pharmaceutical care, and will rotate around different specialities in the Trust. You will have excellent communication, IT and team working skills and self-motivation to contribute to the development and delivery of the pharmacy service within the Trust. This role will involve development of rotational band 6 diploma pharmacists and trainee pharmacists

Main duties of the job

Main Tasks - Trust role

  • Support the Deputy Clinical Director Pharmacy in setting the strategic direction for the Clinical Pharmacy Service
  • Contribute to the development of the annual business plan and strategic planning for the Pharmacy Directorate
  • Implement Trust policies in relation to the Clinical Pharmacy Service
  • Continually review the quality and standard of service provision through consultation with users of the Clinical Pharmacy Service
  • Assists Directorates and specialties in controlling drug expenditure
  • Ensure the appropriate stock control and storage of materials and safe maintenance of equipment used in the provision of the Clinical Pharmacy services
  • Participates in weekend. public holiday and out of hours provision of pharmacy services

Main tasks - Lecturer Practitioner role

  • Supervise student placements
  • Act as mentor for students in practice placements
  • Contribute to recruitment activities
  • To liaise between the Trust and the University for placements and workplace learning activities
  • To take an active role in liaison between CHFT and the staff and students of the University to support relevant research project
  • Design teaching material and deliver across a range of modules within the subject area, using appropriate teaching, learning support and assessment methods
  • Design, mark and give feedback on appropriate assessments
  • Contribute to the revision and improvements in curriculum content and delivery

Clinical Duties

  • Participate in the Clinical Pharmacy Service, reviewing prescriptions for clinical appropriateness, drug interactions and legality; dispenses and supplies drugs for patients
  • Provide professional and clinical information and advice to the multidisciplinary healthcare staff (including primary care), patients and carers, to support individual patient care and medicines optimisation
  • Make recommendations tailored to the context of specific patients or situations, including calculations for complex drug regimens
  • Address issues of co-existing medical diseases and conditions such as renal and hepatic impairment, pregnancy, breastfeeding and concurrent medications
  • Make recommendations in high-risk areas of prescribing, including use of medicines in pregnancy, breastfeeding and intravenous medications
  • Apply sound professional judgement to complex clinical issues and dilemmas, particularly where definitive information is lacking
