Main duties of the job

The post holder will work alongside 2.93 Consultant old age psychiatrists and MDT staff to provide the services to the Newbury locality. The advertised post is predominantly a memory clinic role but the Consultants work flexibly across the memory clinic, CMHT and HTT.

Job description

Job responsibilities

To work as part of the wider team ensuring the provision of a high standard of care for patients under Memory Clinic and potentially HTT and CMHT.

To be a member of the multidisciplinary memory clinic team available to the team for consultation advice and to undertake memory clinic work as it arises including responding to duty queries and supporting MDT staff.

The role will largely include assessing new patient referrals to memory clinic and (with support as needed) to investigate, diagnose and treat these patients.

Investigations will include referrals for blood tests, brain scans, S&LT specialist assessments, neuropsychometric assessments and OT assessments. Onward referrals may include to neurology, elderly care medicine or community neurorehabilitation service.

The treatment will include initiating memory clinic drugs where appropriate as well as referring for CST brain exercise, Understanding Dementia Course, OT, S&LT, Psychology, Dementia Care Advisors, Social Services, Third Sector. etc.

Follow up of memory clinic patients will usually be done by memory clinic nurses unless there is particular (medical) complexity.

To work alongside the service manager and Consultant colleagues to ensure the effective functioning and development of the service.

The postholder would not normally act as care co-ordinator.

To provide information and guidance for patients and carers.

To contribute to in house training, team and peer supervision.

To provide up to date clinical reports on patient care to all relevant agencies involved in the care planning as required.

To keep the computerised clinical record on RIO up to date.

The post holder will fulfil the requirements for the Mental Health Act, Deprivation of Liberty Legislation and Mental Capacity Act and facilitate best practice for vulnerable older people.

The post holder undertakes, exceptionally, to perform additional duties in occasional emergencies and unforeseen circumstances
