Job summary

  • Provide expert clinical support and leadership to community nursing staff working within theWindsor & Maidenhead Community Nursing Service.

  • This role will consist of a minimum of 60% daily clinical patient facing activity.

  • To work flexibly over 7 days to meet the changing needs of the One Community Nursing service.

  • Collaborate closely with the Operational Team Lead to ensure staff competency, including overseeing induction processes and ongoing clinical development.

  • Champion evidence-based practice to ensure delivery of high-quality, safe, and effective patient care.

  • Act as a senior clinical resource, offering enhanced support and guidance for complex patient cases.

  • Promote and support initiatives aimed at reducing inappropriate or unscheduled hospital admissions andfacilitatingtimelyhospital discharges.

Main duties of the job

  • Clinical Governance & Care Quality

  • Ensure care delivery is based on sound clinical assessment and robust care planning processes.

  • Monitor and review staff practice regularly tomaintainhigh standardsof clinical care.

  • Investigate complaints, safeguarding concerns, and serious incidents,identifyingroot causes and implementing improvements.

  • Add areas of concern to the risk register anddisseminatelearning across the team.

  • Leadership & Team Management

  • Exercise leadership and sound judgment insupportingthemanagementteam.

  • Support recruitment and retention strategies to build a resilient and skilled workforce.
