Job summary

Provide expert clinical support and leadership to community nursing staff working within theWindsor & Maidenhead Community Nursing Service.

This role will consist of a minimum of 60% daily clinical patient facing activity.

To work flexibly over 7 days to meet the changing needs of the One Community Nursing service.

Collaborate closely with the Operational Team Lead to ensure staff competency, including overseeing induction processes and ongoing clinical development.

Champion evidence-based practice to ensure delivery of high-quality, safe, and effective patient care.

Act as a senior clinical resource, offering enhanced support and guidance for complex patient cases.

Promote and support initiatives aimed at reducing inappropriate or unscheduled hospital admissions andfacilitatingtimelyhospital discharges.
Main duties of the job

Clinical Governance & Care Quality

Ensure care delivery is based on sound clinical assessment and robust care planning processes.

Monitor and review staff practice regularly tomaintainhigh standardsof clinical care.

Investigate complaints, safeguarding concerns, and serious incidents,identifyingroot causes and implementing improvements.

Add areas of concern to the risk register anddisseminatelearning across the team.

Leadership & Team Management

Exercise leadership and sound judgment insupportingthemanagementteam.

Support recruitment and retention strategies to build a resilient and skilled workforce.
