An exciting opportunity has become available for a Community Staff Nurse to join the Bracknell Community Nursing Team.
Community nursing is evolving, with patients presenting with increasingly complex and acute needs. Our highly skilled team delivers specialist, evidence-based care to patients in their own homes, supporting them to remain independent and receive high-quality care in the community.
In this role, you will assess, support, and treat patients with a wide range of health needs, delivering compassionate and person-centred care.
We are a friendly, supportive team seeking motivated NMC registered nurse who are passionate about delivering excellent community-based care.
As a Community Staff Nurse, you will play a vital role in ensuring patients receive safe, effective, and high-quality care within their own homes.
No two days are the same in community nursing. You will work with patients with varied and complex needs, including wound care, long-term conditions, and end-of-life care, making a meaningful difference to patients and their families.

Main duties of the job

Towork in conjunction with theTeam allstaff within the team to holistically assess the needs of patients and carers, and to work in partnership with the patient and family to plan, implement and evaluate programmes of nursing care.

Tobe able to undertake first visits and take any immediate action, reporting back to the Team Leader/Community Nursing Band 6/Community Matron for further discussion and/or assessment.

Toundertake practice assessor and supervisor training support students andnew staffto the service.

Provide care according to the available evidence of best practice and the support of others in the development ofevidence basedpractice.

Tobe actively involved in promoting self-care for patients and building Health Promotion into care.

Todevelop andparticipatein planned teaching programmes for patients.

Todemonstrateknowledge and skills including assessment, reassessment, medication management, bladder bowel care, tissue viability, care,useand administration of treatment using specialist equipment, palliativecareand symptom control, managing acute episodes of care, reducing hospital admissions, as per band 5 competency document.

Toprovide care to people with long term conditions undertaking reviewsin accordance withthe individual patient care plan and with support of more experienced colleagues asrequired.
