Community Nursing Sister/Charge Nurse
- Location: Wokingham
- Working Hours: Monday to Friday, 8:30 am to 4:30 pm (part-time options available). Includes one weekend every four weeks, bank holidays on rotation, and one late shift (4:30 pm to 7:00 pm) per week as part of a rota.
- Salary: £39,205 to £47,084
We are seeking an experienced Community Nursing Sister/Charge Nurse to join our Wokingham Community Nursing Services team.

- Provide leadership and oversight to the team, supporting and mentoring junior staff.

- Act as a role model, fostering learning and development within the team.

Main duties of the job
- Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
- Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
- Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
- Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
- Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.
