Job summary

Dietitian (Band 6), to work 15 hours per week within the Community Teams for People with Learning Disabilities.

The work is varied and often complex. You will work with a diverse population in both urban and rural settings providing dietary advice based on identified needs, to adults who have a learning disability within any community setting. The post holder will also provide advice and support to families/carers and other provider agencies and their employees.

Main duties of the job

To assist in providing, maintaining, and developing the Nutrition and Dietetic Service for adults with a learning disability living in the community across Berkshire.

To plan, manage and prioritise own clinical workload (including enteral feed patients)and share supervision of the work of dietetic assistant as required.

To plan, co-ordinate and deliver nutrition/dietetic training for professional and support care staff.

To work in liaison with medical, nursing, Allied Health Professionals, Primary Healthcare Teams, social workers, clerical and other relevant staff.

To provide support and cover for other members within the Nutrition and Dietetic Learning disability department, which may necessitate travelling to other locations across Berkshire.

To undertake an assessment of patients' needs including the calculation of nutritional requirements based on the interpretation of biochemistry, anthropometry, clinical condition, and other physical and social parameters.

In collaboration with the patient, carer, members of the clinical team and other colleagues as necessary, to develop a care plan to meet nutritional needs and formulated goals.

To refer clients and their carers to other health professionals and agencies when necessary using the appropriate channels of referral.
