Our supportive and friendly Intensive Community Rehabilitation Team are looking for a registered nurse to provide patient-centred care and rehabilitation to clients in their own homes.

You'll have a chance to play an integral part in patients' recovery after they are discharged from hospital as well as share your knowledge when managing and mentoring more junior team members.

This rewarding and varied role is based at Upton Hospital in Slough, and will also involve visiting patients in their homes (around Slough, Windsor and Maidenhead). Part-time applications that cover core hours (8am - 4pm four days a week) will be considered.

Main duties of the job

  • Perform assessments and health checks in patients' homes following their discharge from hospital

  • Provide therapy-based care and advice to aid patients rehabilitation

  • Implement innovative and structured care in a clinic setting

  • Ensure patients are supported in their preferences and that their dignity is maintained

  • Train, manage and mentor junior members of the Community Rehabilitation Team
