Job summary

A Monday-Friday 8am - 4pm working pattern is just one of the attractions this opportunity provides. We are looking to recruit a Band 6 Community Heart Function Nurse Specialist based at The Coley Clinic in Reading. You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for case management of patients with heart failure in a community setting.

Working as part of a wider integrated Cardiac and Respiratory Specialist Services (CARSS) covering the Berkshire West locality you will work to improve the lives of patients with Cardiac and Respiratory conditions. You will be responsible for ensuring high quality evidence-based care for patients with heart failure, their families and carers in the community

Main duties of the job

Facilitate and over see community-based Heart Failure clinics in various community settings including the patient's home

Work flexibly in response to service need across multiple sites in Berkshire West

Take responsibility for case management of patients with heart failure in the community setting

Supporting colleagues in the delivery of compassionate,safe and structured care

Make differential diagnosis within level of competence

Plan and provide treatment in collaboration with others as appropriate

  • Completed/completing first level degree
  • Heart Failure qualification e.g. Optimising Health & Wellbeing for the Patient with Chronic Heart Failure (Caledonian University) or similar / or willingness to undertak
