Job summary
We are seeking to recruit experienced mental health practitioners to be an integral part of the Mental Health Integrated Community Service (MHICS) for the Maidenhead Primary Care Network (PCN). The post holder(s) will be responsible for working closely with General Practitioners and the primary care team to provide biopsychosocial assessments, brief interventions and care navigation for people with significant mental health issues living in the locality.
This is an exciting opportunity to be involved in the development and shaping of a major new and innovative approach to providing mental health care within the community, working closely with all relevant providers within the individual's local community network.

A ground-breaking new model is being implemented within Berkshire Healthcare NHS Foundation Trust and Surrey and Borders Partnership NHS Foundation Trust to benefit those patients within the primary care setting.

Each PCN, comprising a population of approximately 30-50,000 patients, hosts a multi-disciplinary team which will work in close collaboration with a wide range of stakeholders to provide support for people with significant mental health difficulties to meet their needs, closer to their communities and homes.
