Job summary

Following a review and re-structure of In-Patient Services we have an opportunity to recruit a Band 7 Discharge Co-ordinator for complex cases at Prospect Park Hospital to be a part of our new senior leadership team. The role could be offered as a Developmental Band 6 into a Band 7 role, depending upon previous appropriate experience.

The Bed Management Team has a good core group of experienced staff who proactively manage the referrals into and out of the unit from across the Berkshire system, working across multiple agencies and healthcare providers externally and Trust wide services internally.

We are looking for a capable, reliable, highly organised, warm, and dynamic registered healthcare professional who has previous experience of working at Band 7 level and in managing a complex caseload and has extensive community experience. As the Discharge Co-ordinator you will work closely with the Bed Managers, the Inpatient Services Senior Leadership Team, Multi-Disciplinary Teams and Stakeholder providers to support the development of comprehensive discharge plans for patients with complex healthcare and social needs. You will both maintain and improve the high standards of patient care in these areas, and support and develop a cohesive pathway for patients on your caseload. We are keen to recruit a clinician with excellent communications skills and a proven track record of developing good working relationships across multiple services and settings.

Main duties of the job

  • Work closely with key stakeholders within the Trust - delivering an assertive approach to multi-service work within BHFT, Social Care and Placement Partners.
  • Reach across boundaries and deliver a connected approach to discharge patients with complex presentations.
  • Facilitate the effective flow of patients into the right service at the right time and prevent long delays to discharge.
  • Develop a seamless interface between BHFT colleagues, bringing key decision makers together to identify barriers to discharge and work in a positive MDT approach.
  • Enabling effective and holistic discussions to reach decisions for the discharge of complex patients from inpatient psychiatric services and out of area placements.
  • Working creatively with patients, their families and carers as well as liaising closely within ward MDT's, bed management, Nurse/Practitioner Consultant Network
  • Move On Coordinator, Physical Health Leads, Drug and Alcohol Lead, Psychologist, Pharmacist, Inpatient operational managers, community mental health teams across BHFT and partner agencies.
