Job summary
Are you already a Band 5 Community Nurse or perhaps you are newly qualified / about to qualify and are looking for a flexible, part time role to coincide with you home life whist also paying HCAS?
We are currently looking for a Learning Disabilities Nurse to join our innovative, supportive team here in Erlegh House Reading for 22.5 hours per week.
We are welcoming applications from Students / newly qualified Nurses and offer a preceptorship programme in addition to standard probationary period.
In this role you will have the opportunity to learn about dementia screening and epilepsy care planning in addition to specialist learning disability assessments. You will be supported to develop further in your skills by our friendly team of senior nurses.
You will work with a diverse population in both urban and rural settings providing Community Nursing based on identified needs, to adults who have a learning disability within any community setting. You will also provide advice and support to families/carers and other provider agencies and their employees.
In return we are looking for someone who has a real passion for working with people with Learning Disabilities and who wants the opportunity to make a difference to the lives of people in the community.
Main duties of the job
- To manage under supervision a caseload of clients who have been accepted as referrals to the Health Team
- To contribute to comprehensive specialist nursing assessment that takes into account the nature of the learning disability and its implications for care, to include the monitoring and evaluation of programmes of care specific to the persons health needs
- To promote and facilitate access to primary and secondary healthcare for individuals with a learning disability including health checks and health action plans
- To provide specialist advice and therapeutic interventions to parents and carers of people with a learning disability to enable them to support the person with all aspects of their health and behaviour associated with their learning disability
- To use specialist knowledge effectively to make appropriate referrals to other professionals
The 4 "must haves" for you to be considered for this role:
- Registered Nurse with current registration with the NMC
- Highly developed interpersonal skills
- Ability to work autonomously
- Full UK driving license and access to vehicle
