Job summary
The In Reach Patient Flow Coordinator is a key part of the In Reach Service, working in a patient facing, ward-based role at the Royal Berkshire Hospital. They will work under the direction of the In Reach Team and in conjunction with ward multi-disciplinary teams (MDT). They will assist with the planning and implementation of patient discharge to Community Hospital, as well as aiding general flow of patients through the hospital.
In Reach service is operating Monday to Sunday between 0800-1800 on weekday and 0800-1300 on the weekend.
Main duties of the job
- Liaise with a number of stakeholders to ensure measures are in place to facilitate a safe discharge.
- To have an overview and understanding of the capacity of the teams and the community beds to improve the flow into the community services helping to reduce delays and hospital length of stay.
- Involvement in difficult and complex conversations regarding capacity with service users and stakeholders, including senior clinicians in a professional manner, responsible for delivering bad news if demand outweighs capacity.
- To be the point of liaison with all professionals involved in service users care including primary, secondary, South Central Ambulance Service, community services, carers, and other relevant groups.

- Good general education including GCSE English and Maths (or equivalent)
- NVQ4 or NVQ3 or recognised certificate in office administration or equivalent experience
- Experience of providing clinical support in a health or social care environment at Band 3 or above
- Experience of multi-professional working including MDT care planning.
- Able to lead a process of care planning, pulling together complex information from a variety of sources and creating a shared document.
