Job summary

Are you a dynamic, innovative Physiotherapist interested in Community Stroke Rehabilitation? Would you like support from established Specialist Physiotherapists as well as an integrated multi-professional team to progress your skills and achieve excellence for your patients?

This is an exciting 12 month fixed term opportunity for a part time Band 6 Physiotherapist (22.5 hours a week), within the East Berkshire Earlier Supported Discharge team for Stroke Rehabilitation. The post has an office base at St Mark's Hospital, Maidenhead, clinical care will be provided in patients' own homes within East Berkshire but some cover to the West of Berkshire may also be required. The successful candidate must be a car driver.

In this diverse role the successful candidate will deliver high quality Physiotherapy assessments and rehabilitation for patients who have been discharged from hospital following a stroke, related to their individual goals and needs. The post holder will receive supervision and support for their caseload from experienced Physiotherapists in the team and from the wider Community Neurological rehabilitation service.

We are looking for an HCPC Registered Physiotherapist who has excellent communication skills and a strong interest in stroke and rehabilitation. The successful candidate must be able to work as part of a team but also have relevant experience of independently visiting patients in their own homes.

Main duties of the job

  • You will complete specialist Physiotherapy assessments as well as holistic overview assessments of the patients' needs on behalf of the Multidisciplinary team following relevant induction.
  • Develop knowledge and clinical competence within stroke and rehabilitation and utilise this to set up and deliver excellent Stroke Rehabilitation with the support of other Physiotherapists and the Multidisciplinary Team, delegating to Therapy Assistants as appropriate
  • Use excellent communication skills to understand patient's goals and priorities for rehabilitation, you will engage and motivate Stroke Survivors and their families to engage in rehabilitation, as well as liaise with relevant professionals within the team and in wider services such as GPs, Social Services and the voluntary sector
  • To complete relevant administrative duties such as accurate and concise documentation of notes, reports and data recording as required by the role.

  • Car driver with access to a vehicle to travel to patients' homes across Berkshire
