Job summary

Are you a passionate and driven Mental Health and Wellbeing Practitioner (MHWP) ready to make a difference in a flexible role? An exciting opportunity has arisen as part of the East Berkshire Mental Health Integrated Community Services (MHICS) Transformation programme.

The post holder will be a qualified Mental Health and Wellbeing Practitioner and will work alongside service users, families, carers, and work as part of a multi-disciplinary team to co-ordinate care, supporting collaborative decision-making about care and treatment within one of the East Berkshire (Bracknell, Windsor, Ascot, Maidenhead (WAM), or Slough) MHICS Teams.

Working as part of the MHICS Teams across East Berkshire, you will work with people experiencing a range of Significant Mental illnesses (SMIs) and emotional challenges. This can range from people with lower-level mental health support needs to people with complex lifestyles often coexisting with emotional and mental wellbeing challenges.

The post holder will work under case management supervision from a suitably qualified professional, and work under close clinical skills supervision from a HCPC registered clinical/counselling psychologist or BABCP accredited cognitive behavioural therapist, to provide high quality collaborative care planning as a member of the multi-disciplinary team.

Main duties of the job

  • Carry out relevant risk assessments and risk management with the multi-disciplinary team.
  • Engage calmly and with sensitivity and empathy to support service users in highly distressing or emotional circumstances.
  • Engage with and gather information from service users, relatives, and patient records to develop a formulation.
  • To deliver specified wellbeing-focused psychologically informed interventions, in line with best available evidence, under close supervision from a clinical/counselling psychologist or CBT therapist.
  • To be responsive to service users' needs and choices; and uphold their right to be treated with dignity and respect.
  • To include carers and families in line with the service user's wishes
  • To liaise with other health and care providers, including third sector agencies and primary care, to ensure continuity of care for service users.
  • To communicate information concerning the assessment, formulation, and care plans of service users in a skilled and sensitive manner to promote effective multi-disciplinary working and therapeutic outcomes for clients.
  • To collaboratively co-produce safety plans with service user
  • To develop collaborative plans for relapse prevention
  • To deal with endings appropriately and safely with service users, families, and carers
