Job summary
The successful candidate (s) will be working alongside an established multi-disciplinary team consisting of senior specialist community physiotherapists, Occupational Therapists, Triage coordinators, Nurse, Therapy assistants and team administrators.
As part of this role the post holder will be an integral member of the Triage team. This responsibility is a pivotal post for Intermediate Care patient flow. This will include the coordination of care for when a patient is referred into the service; either discharged from an inpatient unit or referred from a community setting.
The post holder will attend the MDT meetings and allocation meetings ensuring that all referrals are placed in a timely fashion maintaining a seamless referral process for all patients. The role also involves a patient facing elements, this varies on the capacity of the triage coordinators and can be adhoc or more formalised structure depending on service need and staffing.
This part of the role can include supporting the delivery of care, completing initial assessments and identify patient centred goals, progression of care/ therapy plans and delivery of reablement within the patient usual place of residence
Main duties of the job
To be an integral member of the triage team - to assist with the management of referrals, to supporting the integrated triage process and Sitrep flow calls
To ensure that patients taken on through the service are managed in accordance with a clear care plan identifying their needs and ensuring appropriate actions are taken.
You will provide quality reporting service, statistics and audits in line with Government and Trust requirements.
To support the system calls and liaising with partner organisation to identify the most appropriate pathway for the individual to be supported in.
To be able to carry out first/make safe visits to improve patient flow and safe discharge of clients in the community.
To provide personal care and support as required by the care plan.
To discuss patients progress while on the service and review with the multi-disciplinary team as appropriate.
To assist the patient to recognise when their personal agreed goals have been achieved.
The "must haves" for you to be considered for this role:
- An NVQ level 3 or higher qualification or an equivalent.
- Minimum experience of 3 years in the community services supporting patients in their homes/place of residence.
- Car driver with ability and readiness to travel to multiple sites
- Ability to use MS Excel, Teams, Word and Power point systems effectively
