We are looking to recruit an Senior Nurse/ Senior Practitioner, who will have responsibility for the day to day management and clinical leadership of the Urgent Community Response & Virtual Ward team across West Berkshire
The role involves working closely with service users, community-based community services, Primary care, other community health services and social services and the voluntary sector to ensure that the services provided is of a high quality, responsive to local need and demonstrates value for money
You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.
The post holder will work 37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am - 8pm.
The role is based at Hillcroft House , Thatcham and staff parking is available
Main duties of the job
- The post holder will have responsibility for the operational management and clinical support to the Urgent Community Response & Virtual Ward team covering West Berkshire.
- The post holder will ensure that the services are effective and well-co-ordinated, meeting the requirements of the relevant service specifications, whilst keeping in line with CQC requirements.
- The post holder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner
- To provide leadership and line management to the Band 8/7/6 nurses and therapists and the Band 3 & 4 healthcare/multi-therapy assistants.
- Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC /HCPC, information governance and local / national guidance.
- Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.
- Work flexibly in response to service need across our multiple sites
