The post is for an Advanced Nurse Practitioner who will deliver specialist clinical practice within the ARC.
The service aims to deliver care 5 days a week 08.00-16.00.
The role will include clinical triage of patients referred for urgent community care, this may include signposting referrals to other community services, or visiting patients to avoid an acute admission. There is also the facility to step patients up to community inpatient beds.
In the ARC outpatient clinic facility the ANP will undertake holistic advanced history taking and assessment, including diagnostic testing whilst working alongside the Consultant and wider team to create and deliver a management plan.
The post holder will provide advanced clinical assessment skills to patients in ARC clinics to holistically assess and plan care, the patient group may include those who are frail, at risk of falls, have a long-term condition or palliative diagnosis.
Establish, co-ordinate and deliver direct clinical care across the service.
The post holder will also work in partnership with the Nursing Team, Consultant Geriatricians, ward doctors, GP's, Community Matrons, therapy teams, the acute trust, social care staff and the wider multidisciplinary team to provide appropriate patient focused care.
Main duties of the job
- To receive and triage Urgent Community referrals from variety of referral routes and priorities and allocate the referrals based on the clinical needs and utilise the skills mix of available community teams
- To practice at an advanced level of nursing working as an autonomous practitioner within the multidisciplinary team including visiting patients in their own homes, seeing them in clinic, or admitting patients to community inpatient units in collaboration with the specialist doctor, Community Geriatrician and GP.
- As an experienced advanced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment, and management of complex patients considering local and national guidelines.
- Develop, deliver, and ensure high quality innovative clinical practice. Be proactive in clinical decision making, underpinned by an advanced level of theoretical and practical knowledge and be able to demonstrate improved patient care outcomes.
- To be mindful of patients' needs, wishes and culture encouraging self-management. Empower and educate patients, carers, and relatives so that they make informed decisions about their plan of care and encourage self-management.
