We are currently recruiting for a Community Nursing Sister/Charge Nurse to join our busy yet rewarding team.

We are seeking to recruit someone with previous experience of working in a Community Nursing team who can provide leadership and support to develop staff in reaching their full potential.

You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community.

You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community.

Main duties of the job

o Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.o Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.o Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.o Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.o A key member of the Community Team, providing robust clinical services for patients, maintaining positive communication links with the Brookside and Chalfont GP practices and other members of the Multi-Disciplinary Team, providing high quality patient focused care using evidence-based practice.o Participating in the delivery of highly skilled nursing care to patients, providing End of Life Care, wound and IV-line management and assisting patients to develop self-management skills.o Your working hours would be mostly Mon to Fri, 8.30am to 4.30pm (with approximately one weekend every 4-5 weeks and bank holidays in rotation). The Community Nurse service will also be covering the 4:30 to 19:00 pm shift on a Rota basis.
