Job summary
A fantastic opportunity has arisen to be part of a well-established Community Matron and Community Nursing team in Wokingham. The post holder will work full time within an integrated team to proactively manage a complex caseload of high-risk patients, applying advanced clinical skills, clinical reasoning and practising autonomously. The post holder will promote shared decision making with individuals regarding their care plan and encourage proactive self-management of long-term conditions with the aim of avoiding crisis resulting in unplanned hospital admission.
The post holder will work closely with the Community Nursing Teams and other stake holders to manage complex patients as well as the wider multi-disciplinary team in a variety of settings to improve health outcomes and quality of life for patients with multiple chronic diseases.
