Job summary

Royal Berkshire Hospital Psychiatric Liaison Team is an established CORE 24 service as outlined by NHS England. The team is also accredited with the Royal College of Psychiatrists.

The role of the Advanced Mental Health Practitioner within this service is to provide an assessment service to people who are referred to the psychiatric liaison service from the Emergency Department and from the acute admission wards at Royal Berkshire Hospital. The inclusive age range is 16 years and above.

The post holder will be responsible for the formulation, execution and evaluation of a care plan, including signposting to other services. The post holder will ensure that each individual plan is developed in consultation/agreement with the service user and informed by the wider multidisciplinary team.

The post holder will operate within a multidisciplinary team and will be responsible for the delivery of care, risk management and evaluation of care, emphasising individual service user's recovery.

The post holder will be confident in taking positive risk in accordance with the Trust Policy around Risk Assessment and management.

As an experienced mental health practitioner the post holder will direct and supervise senior mental health practitioners and/or students and resources within their area of responsibility and take an active participation in clinical quality assurance and learning programmes.

Main duties of the job

About the Jobo The post holder will be a member of the Psychological Medicine Service and as such will be working across the hospital, covering shifts over a 24 hour, 7 day a week operation as set out in its operational policy.o The post holder will provide assessments to patients 16+yrs, following appropriate induction and training.o To maintain and develop relationships with professionals external to the area of service, service users and their significant others (where appropriate) within area of service/specialism.o To be responsible for the assessment, planning, implementing and evaluation of specialist care, including evaluating the work of junior staff. And to be responsible for development and delivery of intensive crisis care plan, working collaboratively with multi-agencies to ensure service users' needs are met.o To formulate and design physical interventions as part of the service users care plan, ensuring that clinical policies and procedures are followed correctly and risks inherent in any procedures are minimised
