Job summary
This is an exciting new opportunity to work as part of an in-reach team, working closely with the community inpatient units to support appropriate, safe, and timely discharges from the acute to our community beds.
The role will be based initially in the Royal Berkshire Hospital, and you will be working with the wards to ensure the referrals are suitable for a community hospital, have the correct information for a safe discharge and to support the acute wards with their understanding of the patients accepted to a community hospital bed.
This is a senior nurse role, where you will be responsible for identifying and evaluating those patients referred by the acute wards for their suitability to come to a community inpatient setting.
Collaboration and effective engagement is essential to the systems flow work and this role will be an essential component of the effective flow of patients to our community inpatient wards.

Main duties of the job
o To comprehensively assess patients with complex physical, mental, and social care needs using advanced clinical examination skills, detailed history taking and biometric monitoring and evaluation skillso To have an understanding of the combined impact of all of the patient's conditions on their health and social care needs in order to improve health outcomes and quality of lifeo To identify and plan preventative measures and anticipatory care needs and develop crisis escalation (admission avoidance) planso To use independent non-medical prescribing skills and knowledge to facilitate treatment in a professional and timely manner for conditions within the scope of the individual's competencies in line with national legislation and local trust policies.o Use knowledge and skills to order and interpret tests and investigations and to refer to other specialists in a timely fashion as the patients' individual needs dictateo To work in collaboration with patients and carers, identifying self-management needs and increase their knowledge of their condition, in order that they develop skills to manage their condition effectively.o To establish and maintain effective communication and liaison with the Acute Sector, Primary Health Care Team and other agencies involved in caring for patients in the Community i.e. voluntary and statutory agencies, relatives and carers
