Diabetes Technology Lead Specialist Nurse

Job summary

The post holder will work closely with the specialty clinical leads and Consultant Nursewithin the diabetes team to support the effective clinical strategy and operational management for the insulin pump and CGM part of theservice, ensuring a high quality patient care within the resources available.The post holder will be integral to the development and improvement of the pump/technology service along with general support within the team.The post holder will be a, highly visible professional who is approachable to individuals and carers and staff within the Diabetes team. To work as an autonomous practitioner within agreed protocols and to deliver a specialist technology service to patients with diabetes and theirfamilies in conjunction with the lead clinicians and specialty team.

Main duties of the job

Perform specialist assessment of patient's clinical needs, plan, implement and evaluate care delivery using advanced clinical knowledge, clinical examination and clinical skills for those individuals who will be managed by insulin pumps and continuous glucose monitoring .To be clinically credible in the area of Diabetes and other aspects of these diseases.Contribute towards a multi-professional approach in the management of patients, especially in relation to ongoing care needs and discharge arrangements.Support individuals and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.Recognise changes in someone's condition that require the intervention of others and refer on as appropriate.Review medication regime, ensuring that individuals receive appropriate medication in effective doses, and monitor their clinical status following medication changes.To ensure that nursing is underpinned by evidence-based practice where applicable.Establish and maintain effective communication with individuals and carers / relatives.Ensure seamless transition of care from paediatric to adult services and from secondary to primary careProvide accurate discharge information for GPs and community teamsto facilitate effective follow up care and management for those whoaccess the service.Co-ordinate and implement a nurse led outpatient clinic for those requiring ongoing care assessment

As the Diabetes Devices Service Lead Clinical Nurse Specialist, you will be accountable the standards and quality of care delivered within your designated speciality. You will need engagement with all Corporate and Divisional objectives, through strong visible leadership and rolemodelling you will ensure patients are treated with compassion, dignity and respect. You will support the Nurse Consultant /Lead Clinical Nurse Specialist for Diabetes and Endocrine,You will provide professional and clinical leadership within the Diabetes devices service and lead on specific Professional and Service projects asrequired. As a Registered Nurse you will be working at an advanced level undertaking tasks that have been previously undertaken by medical and/or technical staff. As a Practitioner you will practice autonomouslyand also within a defined multi-disciplinary team to provide advice and support to patients, carers, and healthcare professionals on the Diabetes Device Service (insulin pumps and Continuous Glucosemonitoring), across both secondary and primary care and in accordance with local guidelines and policies.You will formulate clinical decisions and treatment plans within local guidelines /policy to manage acute and chronic illness and will respond to actual and potential health problems appropriately. You will undertakeadvanced practice procedures and tasks which will provide holistic nursing care and therapeutic nursing interventions.

The post holder may be required to work on other wards/ departments within the Trust at short notice in the event of hospital pressures/major civilian disaster.
