Job summary

This leadership role provides operational and clinical management to the multidisciplinary Transfer of Care Hub team. The post holder will be responsible for ensuring timely, safe, and effective hospital discharges by coordinating health and social care services. The manager will promote a shared ethos across health and social care professionals, support transformation initiatives, and monitor key performance outcomes, including discharge performance and safeguarding compliance. This role is pivotal to supporting safe, timely discharges and reducing delays across Warrington and Halton Hospitals by integrating health and social care services through robust leadership, performance management, and collaborative working.

Main duties of the job

The Transfer of Care Hub Team Manager is responsible for providing leadership and operational oversight to a multidisciplinary team coordinating safe and timely hospital discharges. Key duties include managing staff performance, rotas, and development; supporting service transformation aligned with national discharge policy; ensuring compliance with safeguarding, clinical governance, and infection prevention standards; and promoting integrated working across health and social care. The role requires strong leadership, communication, and problem-solving skills, alongside knowledge of hospital discharge pathways, the Mental Capacity Act, and system-wide partnership working. The successful candidate will be adaptable, emotionally resilient, and committed to delivering high-quality, patient-centred care through effective team and service management.
