Job description

Job responsibilities

Responsible for the operationalisation of the Emergency and Elective Admission Process across the Trust, in a safe and timely manner and achieving related Trust targets. This requires the ability to balance the demand of an optimal level of elective activity, with a high emergency rate whilst maintaining contingency plans for emerging issues.

To provide Divisions, Directors and external agencies with appropriate information to assess the Trusts capacity and demand.

To provide nursing and managerial support and advice to clinical areas within the Trust, including out of hours.

To facilitate timely re-location of any patients in an appropriate or escalation area.

To have regular communication with Accident & Emergency, Admission Wards and other wards and departments to optimise patient flow.

To contribute to accurate data collection, to ensure correct and timely bed occupancy data.

To have regular communication between teams cross-site to enable a clear picture of bed demand and capacity.

To communicate with ward staff with reference to admission, discharge and transfer of patients.

Disseminate accurate capacity report at the bed meetings and to the relevant Managers.

Liaise closely with the Discharge Planning Team, Rapid Response and other multi-disciplinary teams to expedite patient discharge.

Support the Band 7 Patient Flow Lead Manager to ensure patients are moved to clinically appropriate beds and a safe environment.

Deputise for the Patient Flow Lead Manager in their absence.

Assist in managing patient flow, using Trust Policies and Procedures as guidelines.

Work closely with ward staff, identifying the potential for improving bed utilisation though pathways of care.

The Patient Flow Manager, will act independently, using the Trust Policies and Procedures as guidelines and use his/her judgement to call upon Senior Managers as necessary.

The Patient Flow Manager will work independently and will have the knowledge and experience to judge the situation on a shift by shift basis.

Ensure own compliance and facilitate the compliance of others with regard to Mandatory Training, Revalidation and CPD requirements.

Participate in the education of other staff in relation to capacity and site management, such as staff induction or shadowing.

Provide clinical supervision and mentorship for other staff members.

Maintain professional registration.
