Job summary

In the position of Lead Pharmacist for Acute Medicine, your contribution will be essential in offering pharmaceutical knowledge and guidance within the acute medicine and admissions sector of our hospital.

By collaborating with both the multidisciplinary and clinical pharmacy teams, your responsibility will include guaranteeing the secure, efficient, and prompt provision of pharmacy services to our patients in a hyper-acute and acute clinical setting.

Main duties of the job

  • Collaborate with a multidisciplinary team to optimize acute medical management protocols and patient outcomes.
  • Conduct medication reviews, assess treatment plans, and provide evidence-based recommendations.
  • Educate patients on proper medication usage, self-monitoring techniques, and lifestyle.
  • Develop and deliver educational materials and programs for healthcare professionals and patients.
  • Participate in research activities and quality improvement initiatives related to diabetes care.

Job responsibilities

  • Carry out Lead Pharmacist responsibilities in their area of expertise acute medicine and admissions.
  • Ensure the implementation of the Trust's medicines optimisation strategy for acute medicine and admissions.
  • Conduct risk assessments, uphold clinical governance, and to provide assurance in medicines management across acute medical inpatient services.
  • Engage in ongoing training to keep up with competencies and advance in clinical or technical knowledge within their specialism.
  • Utilise highly specialist knowledge to enhance clinical services, develop protocols or guidelines, conduct audits or research, provide pharmaceutical expertise, and advocate for cost-effective medication use in acute medicine to other healthcare professionals.
  • Manage clinical treatment and medication for specific patients when certified as a non-medical prescriber.
  • Create or modify the weekly service cover rota in the absence of the Principal Pharmacist (or otherwise responsible pharmacist).
  • Address significant operational and clinical pharmacy issues as the most senior pharmacist on site.
  • Contribute to the delivery and advancement of pharmaceutical services at Warrington Hospital, Halton Hospital, and other facilities served by the Pharmacy Department under service-level agreements.

  • Masters qualification in Pharmacy (4 years), or equivalent qualification, plus 1-year pre-registration training and experience.
  • Member of the General Pharmaceutical Council
