Job summary
We have been on an exciting journey to transform the intermediate care services within Halton and have brought together health and social care into a unified integrated team. We are excited to welcome you to join our friendly team where you will have the opportunity to work across 4 areas of the service. Which includes:
- Single point of access
- Community Rapid Response / Frailty Team
- Reablement Team
- Intermediate Care Unit
You will work as part of a multidisciplinary team that includes Advanced Practitioners, Nurses, Occupational Therapists, Social Workers, Care Support Workers and Administrators.
Main duties of the job
We expect you to be hardworking and motivated with excellent clinical, communication, organisational, time management and leadership skills. A flexible approach to day-to-day tasks is essential, in order to meet the demands of the service. You will also be expected to travel across Halton, in line with service needs, so access to transport is essential.
As a B7 Physiotherapist, you will contribute to the clinical supervision and appraisals of students and junior staff. You will participate in service improvement projects, audits, in service training, and work collaboratively with our hospital and community colleagues to ensure our patients receive outstanding care.
The team provides a 7 day service which is currently run with a weekend rota system and rota weekday staffing plan. The working hours for this post will be 37.5 hours - over 5 days which will include a 7-day working contract and contribution to the weekend working rota.
