Job summary
An exciting . opportunity has arisen for a part time physiotherapist 22.5 hours with experience in stroke rehabilitation to lead the physiotherapy service within the Early supported discharge service (ESD) based at Warrington Hospital. The ESD service provides ongoing assessment and rehabilitation across Warrington and Halton working in patients own homes, specialist intermediate care and care homes. You will work closely with Community Therapy services to provide seamless transition of care across the stroke pathway .The successful candidate must be a car driver
Main duties of the job
- You will complete specialist physiotherapy assessments and provide ongoing rehabilitation for patients who have had a stroke who are referred to the ESD team including those with complex needs
- To work closely with Occupational therapy colleagues to provide coordinated care and treatment for patients
- To manage the physiotherapy service within ESD contributing to the coordination of day to day management of the whole service and supporting the clinical lead .
- To provide supervision and teaching to students ,support workers and ESD therapy team
- To drive improvement in clinical and professional practise and performance within the Team
- To act as a source of expertise, demonstrating advanced knowledge and skill in the role and educate other health care professionals as to the therapy role
- To manage data and use to guide improvements in the service
