Job summary
We have an exciting opportunity for a Band 5 Occupational
Therapist willing to engage in a Band 6 training post to work within our Acute
Medical Therapy Team at Warrington Hospital. The successful post holder will
work across the acute medical inpatient bed base, working in a wide variety of specialities
including Frailty, Cardiology and General Medicine. The successful post holder
will demonstrate specialist clinical skills and knowledge in occupational
performance to support discharge planning, functional assessment and provide
therapeutic interventions. The candidate will work closely with the
multi-disciplinary team and community partners to ensure safe, effective
discharges and prevent hospital re-admissions and deconditioning. Our
philosophy is a Home First approach, with an emphasis on supporting patients
personal choices, strengths, and inclusion in every stage of their care. We
adopt an early mobilisation approach which aims to reduce acquired hospital
deconditioning, enabling a person to maintain as much of their independence as
possible.
We are looking for a talented individual who is committed to
working as part of our Therapy Team delivering an excellent, caring service to
patients across our inpatient bed base. Our ideal candidate must be confident,
enjoy working as a part of a large multi-disciplinary team and be able to
demonstrate a desire to actively participate in all aspects of this specialist
role.
Main duties of the job
The successful candidate must be an effective team player
with excellent communication and organisational skills and have a passion for
older people living with Frailty. The
main duties will include taking responsibility to manage a personal clinical
caseload entailing the assessment, treatment and discharge planning of acute
medical patients. It is essential that
the candidate enjoys and is motivated to promote positive relationships with
the Multidisciplinary Team and will work closely with a variety of different
professions to achieve a smooth discharge for patients under their care. The
successful post holder will be expected to provide supervision and support to
junior members of the therapies team and engage actively in quality improvement
initiatives, service development and professional & personal development.
Expectation is that over a 6-month period the post holder
will have training to achieve Band 6 competencies. Once demonstration of
achievement of these competencies, the post holder will become a substantive
Band 6. The working hours for this post are 37.5 hours over 5 days; this
includes covering a weekend rota which does include Saturdays.

This will be done with the guidance and supervision of a senior
therapists with wide support from the therapies team. On completion of the competencies the post
holder will fulfil the role summary detailed below.

post holder will appropriately assess and treat individuals using an integrated
approach to formulate an effective discharge plan and positive patient journey
throughout admission. The therapist will be required to manage a caseload
alongside Physiotherapy colleagues on the acute wards, including some complex
patients. The post holder will demonstrate specialist therapy skills in the clinical

First philosophy. There will be opportunities to rotate within the acute wards,
covering a significant portion of the hospital, to support continued

students and junior staff and will be supported by senior members of the team.
You will participate in service improvement projects, audits, in-service
training and work collaboratively with community colleagues to facilitate
timely and safe discharges, with a key focus on preventing hospital acquired
