We are seeking an enthusiastic and skilled Band 5 Mental Health Practitioner to join our friendly team at Crediton Hospital, supporting the Crediton and Okehampton community.
This is a fantastic opportunity for a motivated individual looking for a fixed term or secondment position to cover maternity leave until 31/10/2026. In this varied and rewarding role, you will hold a caseload of clients with severe mental illness (SMI), delivering evidence-based interventions and providing day-to-day therapeutic support.
You will work collaboratively with an experienced multidisciplinary team, including band 6 colleagues, the team manager, consultant psychiatrist, and psychology professionals. The ability to travel across the area is essential to meet the needs of our clients.

Main duties of the job
- Hold and manage a caseload of clients with SMI, delivering personalised, evidence-based interventions.
- Carry out comprehensive risk and needs assessments, complete PROMS, and contribute to the development, review, and implementation of care plans.
- Offer day-to-day therapeutic support and respond promptly to crisis situations, ensuring accurate and timely clinical record-keeping.
- Liaise closely with band 6 staff, the team manager, consultant psychiatrist, psychology, and the wider MDT to deliver holistic care.
- Conduct physical health monitoring and participate in duty cover as required.
- Champion recovery, independence, and social inclusion for clients within the community.
- Flexibility and the ability to travel throughout the Crediton and Okehampton area are essential for this role.
