Job summary

We are looking for a passionate, skilled and highly motivated Senior Mental Health Practitioner (30 hours per week) to join the Exeter City Core Community Mental Health Team.

The successful applicant will be an innovative and compassionate clinicianwho is motivated to deliver individualised, person-centred care and make a positive contribution to service development. This is an opportunity to join us as we modernise and improve our services to meet the needs of the Exeter population.

You will be an experienced mental health professional (Registered Nurse, Social Worker or Occupational Therapist) who has a genuine interest in collaborative working with people of varying ages and complex needs, including service users, their carers and families.

You will join a rich multi-disciplinary team which includes Nurses, Occupational Therapists, Psychologists, Psychiatrists and Support Workers.

We are forming closer working relationships with colleagues in Primary Care and the Voluntary Sector.

Main duties of the job

With training and support, you will feel confident in delivering therapeutic interventions and liaising with other professionals regarding progress and outcomes. This is an excellent opportunity to learn and embed leadership and management skills.You canprovide supervision as part of this role, as well as supporting the Team Manager with other operational management tasks.

As a core member of the team, you will be responsible for your own caseload and provide assessment, case management and deliver interventions for people with a wide range of needs. You will ensure that the service users recovery goals are central to their Care Plan and promote social inclusion, independence and wellbeing. You will demonstrate excellent communication and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.

You will have an enthusiastic approach towards your professional development. Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff

An understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
CPA Process and Personalised Care Framework.
Substantial experience of managing a extensive and complex caseload of clients with a range of complex mental health problems.
