Job summary

We are seeking a dedicated, compassionate, and highly organized Project Officer to join our team. This pivotal role will be responsible for developing and managing a coordinated pathway of care and support for young people moving through the criminal justice system throughout Devon and Cornwall; The role involves ensuring continuity of care across various services, including youth justice, social services, education, and mental health support. You will work in conjunction with a skilled youth practitioner.

This is a new role which is part of the exciting new Integrated Non-Custodial Services (IN-CS) contract, which has been awarded to DPT for the next seven years and marks a major step forward in the integration of health and justice services.

The IN-CS contract brings together CJL&D, Reconnect and the Mental Health Treatment Requirements (MHTRs), creating an integrated approach to supporting individuals involved in the criminal justice system. From custody diversion through to successful realised and integration into the community. The IN-CS model aims to provide effective care, support and rehabilitation to address both health care needs and offending behaviours. As part of this contract, the Children and Young Persons (CYP) pathway is a vital and integral component.

Main duties of the job

The post holder will have the following key areas of responsibility:

  • Pathway Coordination:
    • Develop and implement individualised care pathways for young people involved in the criminal justice system.
    • Coordinate services across a range of providers, ensuring that young people receive seamless, continuous care.
    • Act as the primary point of contact for young people and their families, offering advice and support throughout the process.
  • Collaboration and Advocacy:
    • Work closely with partner agencies, including youth services, probation, social services, and education providers, to ensure all aspects of the young persons needs are met.
    • Advocate for young peoples needs and ensure they have access to appropriate resources.
  • Monitoring and Evaluation:
    • Monitor progress through the pathway and adjust services as needed.
    • Evaluate the effectiveness of the pathway, making improvements based on feedback from young people and service providers.
