About the role

As a Specialist Clinical Practitioner, you will play a key role in delivering high-quality, person-centered care to individuals across the area, as part of the Additional Roles Reimbursement Scheme (ARRS).

You will work collaboratively with multidisciplinary teams to assess, plan, and deliver services that improve the health and well-being of those in need, whilst promoting recovery and independence.

We are looking for an individual who is committed to providing exceptional clinical care and has a genuine passion for improving outcomes for patients.

You will need to have strong clinical experience and the ability to work effectively within a community setting.

This role is based in Tiverton, a charming market town in the heart of Devon. Tiverton offers excellent transport links, picturesque countryside, and a thriving local community.

Main duties of the job

Key responsibilities include:

  • Providing expert clinical assessments, care planning, and delivery to individuals with a range of health needs.
  • Supporting the implementation of evidence-based interventions to help achieve positive outcomes for patients.
  • Acting as a resource for clinical advice and support to nursing colleagues and other healthcare professionals.
  • Promoting best practice in clinical care and maintaining up-to-date knowledge of developments in nursing.
  • Engaging in multi-disciplinary working to develop effective care pathways for patients within the community.
  • Encouraging patients to take an active role in their care and supporting them to achieve their goals.

Qualifications

Essential

  • Clinical based Degree, supplemented by post graduate diploma specialist training, experience, short courses.
  • A minimum of 60 credits at masters level in relevant subject (e.g. Advanced psychosocial assessment and formulation, Enhancing the Care and Support of the Person with Dementia and their Carer). Or demonstrable equivalent experience/competence at the specialist practitioner level; If a minimum of 60 credits at masters level in relevant subject is not already obtained, this will need to be completed within 2 years of commencing employment as training availability allows.
  • Evidence of Continuous Professional Development.
  • Current registration with the NMC or HCPC.

Desirable

  • Post basic qualification in speciality Application Non-Medical Prescriber (V300) at level 7 (nursing or pharmacy registered only).

Knowledge

Essential

  • An excellent understanding of the NSF, 5 year forward review for mental health and Mental Health Act/ Mental Health Capacity Act requirements.
  • Awareness of community structures and social care options and processes

Skills & Abilities

Essential

  • High level communication skills including: Presentation and facilitation skills; Consultation and engagement skills.
  • Developing short to medium term project based action plans.
  • Ability to travel across Devon and other locations in the course of the business requirement.
