We are excited to announce a fantastic opportunity to join our team as a Youth Practitioner within the Criminal Justice Liaison & Diversion (CJL&D) service, which is part of the newly awarded Integrated Non-Custodial Services (INCS) contract, which has been awarded to Devon Partnership Trust (DPT) for the next seven years and leads to the integration of health and justice services.

We are seeking a dedicated and skilled individual to form and manage a seamless pathway through the criminal justice services for young people in Devon and Cornwall. This role involves coordinating services, ensuring that young people receive continuous care and support throughout their journey.

We are seeking a dedicated, compassionate, and highly organized Practitioner to join our team. This pivotal role will be responsible for developing and managing a coordinated pathway of care and support for young people moving through the criminal justice system.

In addition, the Practitioner will be responsible for delivering training to staff groups. The ideal candidate will have experience working in the criminal justice system, a strong understanding of the challenges faced by young people.

Main duties of the job

We are seeking a dedicated and skilled individual to form and manage a seamless pathway through the criminal justice services for young individuals in Devon and Cornwall. This joint role will involve coordinating services and ensuring that young people receive continuous care and support throughout their journey.

Key Responsibilities:

  • Pathway Coordination:
    • Develop and implement individualised care pathways for young people involved in the criminal justice system.
    • Coordinate services across a range of providers, ensuring that young people receive seamless, continuous care.
    • Act as the primary point of contact for young people and their families, offering advice and support throughout the process.
  • Staff Training and Development:
    • Deliver training to multi-disciplinary staff groups, ensuring they are equipped to provide consistent and effective support to young people.
    • Promote understanding of trauma-informed care and the challenges faced by young people within the justice system.
  • Collaboration and Advocacy:
    • Work closely with partner agencies, including youth services, probation, social services, and education providers, to ensure all aspects of the young persons needs are met.
    • Advocate for young peoples needs and ensure they have access to appropriate resources.
  • Monitoring and Evaluation:
    • Monitor progress through the pathway and adjust services as needed.
    • Evaluate the effectiveness of the pathway, making improvements based on feedback from young people and service providers.

  • Understanding of CYP-IAPT principles including the use of evidence based therapies and Routine Outcome Measures
  • Ability to communicate with a wide range of professionals from various settings, sharing own professional knowledge, skills and expertise, whilst valuing the perspective of others.

Qualifications

  • A professional qualification at degree level e.g. Psychological profession, Nursing, Social Work, Occupational Therapy or a professional qualification with significant relevant experience i.e., Diploma of Nursing with relevant experience.
  • Professional registration/accreditation with a recognised body, e.g. NMC, HCPC, Social Work England, UKCP, BABCP

Desirable

  • Post graduate qualification relevant to specialism
  • Clinical Supervision qualification

Experience

  • Substantial experience of managing an extensive and complex caseload and working with children/young people presenting with a range of complex mental health presentations.
  • Experience of working in a multi-disciplinary and multi-agency environment.
  • Experience of delivering model based psychological therapy
