This is a Fixed Term Contract opportunity for up to 1 year to cover a period of Maternity Leave. A secondment would be considered for any internal candidates.
This is an exciting opportunity to join the Community Forensic Team at one of the Country's most respected providers of secure mental health services.
We are looking for a highly motivated Support Time Recovery Worker to join the team based at Easby House, Langdon Hospital, in Dawlish.
You will support individuals in their recovery journey through secure care and into the community. You will support the caseload to access opportunities to develop their skills, improve their health, wellbeing, and quality of life whilst promoting their reintegration into the community. This role is dynamic and rewarding, and allows and encourages the time to build a therapeutic relationship with our service users.

Main duties of the job
You will become an important part of the team, working to ensure that every patient receives high quality care. You will provide support to both inpatient and community based service user, and have an active involvement in the delivery of a high quality, safe and compassionate mental health service.
You will be supporting the Care Co-ordinators (CCOs) with the assessment and care planning process, including active participation in a range of clinical meetings.
You will aid with initial assessments which may include; gathering information to support this process, support with assessment documentation, contributing to the formulation process and any other related tasks under the supervision (directly and indirectly) of CCO.
You will support the CCOs in the care planning process where directed. This may include; writing care plans for specific aspects of care, gathering information to support this process, support with careplanning reviews and any other related tasks.
Use of specific standardised assessment tools including carers assessments and physical health assessments (under supervision of a CCO). This will include the use of relevant frameworks and documentation for specific clinical areas.
You will contribute to care co-ordination and care management responsibilities including making applications for funding panel for enabling support social care packages, the monitoring and review of a caseload including active case reviews (under the supervision of a CCO).
