Job summary

As a Senior Case Manager, the post holder will be an integral part of the South West Provider Collaborative clinical commissioning function providing clinical, operational and quality oversight in conjunction with the Clinical and Programme Director and Head of Service. The post holder will ensure safe and effective care pathways are in place for all service users in specialised mental health, learning disability and autism care settings, within NHS and independent sectors for the regions population.

Main duties of the job

The post holder will (not exhaustive):

Provide clinical leadership and senior pathway advice to support to quality of care and experience for service users in Secure, CAMHS, AED & Perinatal; this includes holding a small defined caseload.

Manage the group of case managers within SWPC and any other staff deemed appropriate.

Provide clinical support to case managers and local commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.

Work closely with other wider mental health (NHS & Local Authorities) and statutory bodies across the region in terms of pathway management and interfaces as well as supporting service developments and improvements.

Maintain a strong quality & safety monitoring and oversight function for all of the services within SWPC

Support the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery

Define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.
