Job summary
Exmouth Core Adult Mental Health Team are looking for an experienced Band 6 Senior Mental Health Practitioner, based in Exmouth.
This is a substantive full time post, 37.5 hours per week over 5 days, you will be joining Devon Partnership NHS Trust, which encompasses urban, rural and coastal demographics which combine to make this a fascinating and beautiful area to work.
This post is open to applications from persons who are qualified as a Mental Health Nurse, Social Worker or Occupational Therapist.
The successful candidate will be working alongside a dynamic, motivated and skilled Community Mental Health Team.
This post covers Exmouth and the surrounding area.
Working within a clear recovery and social inclusion strategy, the service will endeavour to facilitate personal growth and provide opportunities for personal fulfilment.
We are looking for someone to help build the team further, upon the successful work it has already done following its re-design and with the exciting on -going work linked to the New Community Service Framework & the Primary Care Networks.
Main duties of the job
You will be passionate about making a difference to peoples lives. You will directly support people in their ongoing recovery, managing a defined and complex caseload and providing expert assessment and treatment which fully involves families and carers where appropriate.
The role will include:
- Providing complex assessment and care planning for individuals and their families/carers.
- Working as part of a multidisciplinary team.
- Engage service users and carers assertively in the community.
- To develop and maintain therapeutic relationships with service users and their families and carers and other services to prevent hospital admission.
- Offer a range of NICE evidence based interventions.
- To offer proactive risk management approaches to help the service user and their family.
- Develop and maintain effective working relationships and networks with colleagues within mental health services, primary care and other partner agencies including physical health checks and interventions.
- Provide interventions and work with other disciplines/agencies/teams in the care of service users experiencing co-morbid problems with substance use/misuse.
- Engage the service user and their family in relapse prevention.
- To support medication concordance.
- You will become an advocate for the people in your care, supporting and signposting them, so they can get back into employment or access housing, focusing on their recovery and helping them live the life they want to live.
