Job summary

The post holder will be an integral part of the South West Provider Collaborative commissioning function providing operational and quality oversight in conjunction with the Head of Service. The post holder will work with the Senior Case Managers to ensure safe and effective care pathways are in place for all service users in specialised mental health, learning disability and autism care settings, within NHS and independent sectors for the regions population.

Main duties of the job

The post holder will (not exhaustive):

Deputise for the Head of Service

Work with the senior case managers to provide leadership and senior pathway advice to support to quality of care and experience for service users in specialised services; this includes holding a small defined caseload.

Work with the senior case managers to manage a group of case managers within SWPC and any other staff deemed appropriate.

Work closely with other wider mental health (NHS & Local Authorities) and statutory bodies across the region in terms of pathway management and interfaces as well as supporting service developments and improvements.

Maintain a strong quality & safety monitoring and oversight function for all of the specialised services within SWPC

Support the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery

Define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.

  • Educated to masters level or equivalent level of experience of working at a senior level in specialist areas.
